A way of doing something special to remember and honour a loved one who passes away is to make a donation to Heart-Links in their memory.

To make such a donation In Memoriam, please print and complete the donation form and mail it to Heart-Links. We will send you a receipt for the gift, and we will send a card to the family informing them of your gift.
